only thing i wanna add is make sure all your gear is accounted for before you let anyone know your intentions.. just human nature being what it is and such...

My intention last night was to let the leader know I was leaving after the gig. I arrived early to set up and when the leader arrived, he told me he almost cancelled the gig. His dad is dying and he had been sitting with him all day in hospice. He wasn't sure his dad was going to make it through the night. I didn't think, "sorry about your dad, by the way, I'm quitting the band" was appropriate so I guess I'll wait a little while longer to let him know.
Good call. You definitely need to let some time pass.

More to follow...

Leave amicably.

Give them a date (a month or so), maybe even recommend or try to find a replacement for auditioning.

Tell everyone at the same time, in a group meeting/setting.

Make it "officially" firm, cordial.

No mud slinging

If mud slinging starts, don't get involved. Just stay on the point that you are leaving.

Never say I'm sorry. Remain professionally upright and straight forward.

Stick to the date.
