I agree with the above posts about not stopping part way through a song, unless it becomes a complete train wreck that is.
In my old band we had a few songs where we would play a section on loop...

I usually wear vans or converse...
But I don't tend to own more than two pairs of trainers at a time so it really depends on what I'm wearing/owning at the time.

I've been using ear plugs with filters for a couple of years now and I would definitely recommend some kind of hearing protection. It took a few practices to get used to it but now I can't play or go...

I definitely get a feeling before I play in front of people (I'd call it positive nerves) but I personally wouldn't class that as performance anxiety as it doesn't worry me at all.
