I actually hold sticks so that my thumb is "in between" the index and the middle finger. The three fingers form a sort of a triangle that way. I just developed this naturally without even thinking...

For me, the exercises in Stick Control, pp. 12-15, work wonders. Play it slow, and repeat many times. Also Joe Morello has some great buzz roll exercises in his Master Studies book.

I play the right-handed drum setup and I play the ride cymbal with my right hand. My right hand is weaker than my left hand, but I think I already got to the point where riding with my right hand...
