This is a posture issue. I would suggest taking Pilatest classes and possibly private sessions. It helped me overcome the same issue and it improved my playing significantly.

Great topic! I struggle with myself. I find that I keep time best when I am relaxed, listening, and have really internalized (memorized) the music.

This happens to me all the time. I just constantly strive to improve my concentration. Be sure to always warm-up and memorize your songs. This will improve your consistency.
